Transitional housing is designed to combine accommodation with case management, wellbeing support, practical skills and exit planning. Residents are admitted through an assessment and coordinated-entry process rather than an automatic online booking.
What the shelter workflow can coordinate
- Intake, admission, room/bed assignment and expected stay period.
- Risk and vulnerability information on a strict need-to-know basis.
- Care plans, referrals, appointments, case notes and follow-ups.
- Wellbeing check-ins, mental-health and medical follow-up.
- Gate passes, visitor requests, incidents and safeguarding action.
- Exit planning, discharge or transition into community/alumni pathways.
